You must complete and turn in both forms, Volunteer Opportunities Form and Criminal History Form     

* Print and complete Volunteer Opportunities Form

* Review the Powerpoint Volunteer Program

* Print and complete Criminal History Form

* Return the Volunteer Opportunities and Criminal History Forms to your child's campus 

* You will be notified when you have been cleared to volunteer
